Repeat Prescriptions:

By arrangement with your doctor you may have repeat prescriptions for long term treatment. Either write to us, email (see below) or drop your repeat prescription slip into the surgery. Your prescription will be ready within 2 working days. If you enclose a SAE we will return it to you by post. Repeat prescription requests will not be taken over the telephone.

Please note that when collecting your prescription you must be over 16 years of age or have appropriate written permission.

• Full Name
• Date of Birth
• email address
• Address including postcode
• Telephone number
• Mobile phone number
• Your usual Doctor
• If you wish the prescription to be sent direct to a chemist
• Name of the drug
• Strength
• Frequency

Please do not try to collect your prescription until after 2 working days
