Temporary New Patient Registration Process During Covid-19 Emergency
During the Covid-19 emergency the Sussex Clinical Commissioning Groups have amended local guidance to support the registration of new patients during the current crisis.
To avoid unnecessary access to The Avenue Surgery premises without a pre-assessed clinical need, patients who have recently moved into the areas should now complete the GMS1 online Registration Form (see link below) which will allow you to submit your application direct to The Avenue Surgery.
Please allow up to 7 days for your application to be processed. Only apply once.
Please note: The surgery covers Moulsecoomb, Bevendean, Coldean, Falmer, Hollingdean and the area of Brighton north of Elm Grove (click here for indicative Practice boundary). You may register with the Practice only if you live within these areas.
If you are not moving house outside the boundary of your current practice you are advised to remain temporarily registered with your current practice to ensure that you can continue to receive care from a healthcare professional who has immediate access to your full medical records.
We will contact you when normal service is resumed and ask you to present your ID, sign the GMS1 form and complete The Avenue Surgery’s own New Patient Registration Questionnaire.
