We would like to know how we can improve our service to you and how you perceive our Surgery and staff.
To help us with this we have set up a Patient Participation Group (PPG) so that you can have your say.
PPG members are contacted throughout the year by email (preferred method) or by post (if a member does not use email) to seek their opinion on services provided at The Avenue Surgery as well as other services and agencies. With the support of the PPG we seek patients’ views and draw up each year an Action Plan to improve patient services.
By providing your contact details you are consenting to The Avenue Surgery adding you to the PPG contact list. This will mean that we can contact you by email or mail with regards to PPG business.
You can opt out at any time by emailing:

or phoning the surgery on 01273 604220 and asking to speak to The Practice Manager. At this point your details will be securely removed from the PPG contact list. Your details will not be shared with anyone else.
The information you provide will be stored securely and will only be used for PPG purposes. It will be used lawfully, in accordance with the Data Protection Act (DPA) and the General Data Protection Regulation (GDPR) 2018. The DPA and GDPR give you the right to know what information is held about you and sets out rules to make sure that this information is handled properly.
